To help provide timely service to our patients, all visits must be scheduled. If you are later than 15 minutes for your scheduled appointment you may be asked to reschedule. If the level of volume in the office allows, we will do our best to work you back into the schedule, but there may be an increased wait time. Please remember that this is out of courtesy to all our patients.
Please provide at least 48 hours notice for any cancellation. Missed appointments that were not cancelled ahead will be subject to a cancellation fee.
The office will book consecutive appointments for up to two siblings. Unfortunately, due to scheduling restraints, we are no longer able to accommodate more than this in a single visit.
All new patient forms must be completed prior to your first visit. If you have not filled them out at home, please arrive 30 minutes early to allow adequate time to complete all forms. Failure to complete forms ahead of time may result in a delay of your appointment.
